Surgical Release Form

Prior to anesthesia, your pet will receive a full physical exam.  Pre-anesthetic blood work is recommended and will be completed before anesthesia to evaluate the overall health of your animal. 

13.2 lbs and under up to $25/ over 13.2 lbs up to $36
Local anesthesia and non-steroidal pain medication included with surgery
All patients will receive subcutaneous fluids unless IV catheter is elected.
